Position Summary: The Public Health Nurse (PHN) plays a pivotal role in promoting and safeguarding the health and well-being of the community. They implement and evaluate comprehensive healthcare programs, educate the public on health and wellness, and provide direct care to individuals. Their focus spans beyond individual care, addressing health at a population level.
Key Responsibilities: Community Assessment: Assess community health needs, identify potential risks, and recommend interventions. Program Development: Plan, implement, and evaluate public health programs tailored to community needs. Education: Educate the public on various health topics such as nutrition, exercise, immunizations, and disease prevention. Disease Control: Monitor disease outbreaks, immunize at-risk populations, and participate in disease control initiatives. Direct Care: Provide primary care services such as physical exams, wound care, and immunizations. Counseling: Offer health counseling to individuals, families, and community groups about preventive health measures. Case Management: Manage and follow-up on cases, ensuring that patients receive appropriate care and resources. Collaboration: Collaborate with other healthcare professionals, organizations, and government entities to enhance public health services. Advocacy: Promote health policies and practices that foster individual and community well-being. Documentation: Maintain accurate and up-to-date records, ensuring compliance with legal requirements and confidentiality.
Qualifications: Bachelor's Degree in Nursing or its equivalent. Current Registered Nurse (RN) license in the state of employment. Preferably certified in public health nursing. Previous experience in community health, outreach, or a related field is advantageous. Strong communication and interpersonal skills. Ability to work autonomously and as part of a multidisciplinary team.
Physical Requirements: Ability to stand, walk, and move around for extended periods. Capability to perform clinical tasks such as administering injections, wound care, etc.
Working Conditions: May work in varied environments including health departments, schools, homes, and community centers. May be exposed to infectious diseases. Potential for irregular hours, based on community needs or health crises.
